Bronx toddler critical after being found unresponsive

A 3-year-old girl is in critical condition after being found unresponsive at a city-run homeless shelter, according to police sources.
Police sources say 25-year-old Tiara Cohen couldn't wake her 3-year-old daughter Heavan Wednesday morning so she called 911. Police and emergency crews responded to the Claremont Annex, a city-run homeless shelter where Cohen is living. 
The little girl was rushed to Bronx Lebanon Hospital and then transferred to Mount Sinai in critical condition. Police sources say Heavan is bleeding in the brain and has burns on her arm. They say she is also appeared to be malnourished.
According to police sources, Cohen says Heavan fell at the playground during some rough play three days ago.
The Administration for Children's Services issued a statement saying, "The allegations surrounding this case are very concerning and they are actively investigating the circumstances."
Police say Cohen has four other children. They say they are trying to figure out whether or not the incident was an accident.
