Faraz Kelib, an 80-year-old resident of Hamden, died following a crash on Forest Road. 

Authorities report that Kelib was driving north towards Willard Street when his vehicle swerved onto the sidewalk, colliding with a telephone pole. 

Despite the swift response from first responders, Kelib succumbed to his injuries at the hospital. 

New Haven Police are urging witnesses to come forward with any information.
