New York State Police are reminding commercial drivers to check their brakes after a box truck crashed in Chappaqua.

Authorities say the driver left the truck parked on a hill to make a delivery, only for it to begin rolling forward and strike another vehicle.

Inspectors later found that a critical component of the brake system had dislodged, preventing one of the brakes from properly functioning.

They also determined that the rear brakes were out of adjustment, leaving the vehicle with “the equivalent of only 1.5 properly functioning brakes out of four.”

The truck was immediately placed out of service due to safety violations, according to officials.

State police say this incident should raise awareness about the importance of routine vehicle maintenance and brake inspections.

“Identifying worn or damaged components and correcting brake adjustment issues before they result in a failure helps prevent crashes and protect everyone who shares our roadways,” they stated in a release.
