    Lawmakers say a breakdown led to the death of a mother in Milford last month.
    Top Democrats Monday proposed a bill requiring "timely responses" between police and prosecutors.
    Police say Julie Minogue's husband attacked and then killed her with an ax in December.
    Weeks earlier, Minogue asked police to arrest him for violating a protective order. But that arrest never happened.
    Prosecutors say Milford police never resubmitted an application for an arrest warrant.
    "Trying to understand why, frankly, paperwork doesn't seem to get from one place to another. Do we need to create a law? Do we need to have some more accountability in our system so that breakdown doesn't happen in the future?" said state Sen. Mae Flexer.
