A substance abuse counselor from Norwalk is facing charges for health care fraud and identity theft.

Federal prosecutors say Alan Bradley used patients' Medicaid identification numbers to submit claims for psychotherapy sessions that never happened.

Prosecutors say Bradley was living in Florida when some of the sessions allegedly took place.
