An investigation by the Occupational Safety and Health Administration found that the death of a Plainville firefighter in July was an accident.

Ray Moreau was killed when responding to a house fire on Maria Road July 27.

The investigation found that Moreau was trapped against a truck when a hose became tangled in the wheels of an engine.

A line broke off, which caused him to be struck by a 35-pound intake valve that broke off, launching Moreau about 10 feet.
